We are delighted that you’ve chosen Spine and Orthopedic Solutions for your treatment. To assist with the check-in process for your upcoming appointment, please complete your registration paperwork in advance and bring it with you to our office.
Before your appointment at the SOS: Spine and Orthopedic Solutions, please make sure you have the following items. For any additional questions, call us at 615-730-8626.
- Your insurance card
- Your photo ID
- A list of current medication, with dosages
- Imaging CD
- New patient paperwork
- Ability to cover copayments (we accept cash, card, and check)
- A referral from your primary care physician, if needed
